Neothelids are extremely rare. When Illithids (or Mind Flayers) are created from tadpole-like creatures. When left alone, those tadpoles cannibalize each other until only the strongest one survives, and grows into a massive gargantuan monster hated by all. These chaotic evil aberrations aren't for a low level party. But if your party can take on a neothelid and survive, they'll have some serious stories to tell at the tavern!
